Canine Lymphoma: Symptoms, Diagnosis, Treatment, and Prognosis
Lymphoma is the most common cancer in dogs, but it is not one disease. What swollen lymph nodes mean, how it is diagnosed, and what treatment can really do.
Most owners meet the word lymphoma in the same way: a hand under the jaw during a scratch, an odd firmness where there used to be none, and a sudden interest in what lymph nodes are. Before going further, a piece of context that matters. Lymph nodes swell for all sorts of ordinary reasons, most commonly infection, and a swollen node is not a diagnosis. But it is a reason to be seen fairly promptly, and Morris Animal Foundation's 2023 white paper on lymphoma in pets explains why the timing is worth taking seriously.
Lymphocytes are a type of white blood cell and a core part of the immune system's defences. Lymphoma occurs when those lymphocytes start dividing uncontrollably. Because they originate in blood cells, and blood travels everywhere, the white paper notes that this cancer can spread more readily to many different organs and body areas than a cancer that starts as a lump in one place. That is the central difficulty with it.
It is also common. The paper describes lymphoma as the most common cancer in both dogs and cats, accounting for up to 24% of all cancers diagnosed in dogs. It can affect an animal at any age, but is mostly seen in middle-aged and older ones.
It is not one disease, and that changes everything
For a long time lymphoma was treated as a single condition. Advances in diagnosis have shown that it is not. The white paper reports at least 30 different types of lymphoma identified in dogs, each with its own features and its own response to treatment. Many more subtypes have been described in people.
This is genuinely important for owners rather than a technicality. It explains why two dogs with the same diagnosis can have very different conversations with their vet, why prognosis varies so widely, and why a general survival figure you find online may have nothing to do with your dog. The paper is explicit that prognosis depends on the subtype present, the sites affected, and the stage of the disease at diagnosis. Understanding which subtype is in play has, in its words, major implications for treatment.
What owners actually notice first
The most common form in dogs is multicentric lymphoma, which affects lymph nodes throughout the body and accounts for the large majority of canine cases, according to the MSD Veterinary Manual. The classic presentation is generalised, non-painful enlargement of the peripheral lymph nodes — the ones you can feel, under the jaw, in front of the shoulders, and behind the knees. Nodes affected this way can reach several times their normal size.
- Firm, non-painful swellings under the jaw or in front of the shoulders, often on both sides, that do not settle down over a week or two.
- Nodes that feel enlarged in several places at once rather than just one.
- More general changes as disease progresses: lethargy, weight loss, reduced appetite, weakness.
- Signs specific to where the cancer sits, such as digestive upset when the intestinal tract is involved, or laboured breathing when the chest is.
Breathing difficulty is the exception to the calm advice in this article. If your dog is struggling to breathe, that is an emergency regardless of cause. Go to a veterinarian immediately.
Treatment: what it can and cannot do
Chemotherapy is the usual approach, and the white paper is careful about how it frames the results. It is not a cure-all, but it can extend both the length and the quality of life for dogs and cats. That second half is the part owners tend to be most surprised by. Veterinary chemotherapy protocols are generally dosed with quality of life as the priority, and most dogs tolerate treatment considerably better than people expect from human experience of the same word.
The paper gives a stark comparison for context: without treatment, life expectancy is often just a few weeks to months. Pets with less aggressive lymphoma that receive treatment may go into remission for up to a year, and sometimes longer. It immediately adds the necessary caveat, which is that every animal is an individual and outcomes vary. Your veterinary oncologist's estimate for your dog is worth infinitely more than any number in an article.
There is also a reason relapse is treated so seriously. The white paper explains that lymphoma cells evolve over time, adapting to treatment and sometimes becoming drug resistant, so the cancer that returns after remission is not quite the same cancer that went in. Drug resistance is a significant area of ongoing research, and it is why a second remission is generally harder to achieve than the first. Specific drugs, protocols and doses are decisions for a veterinary oncologist, not something to be researched independently.
What causes it, and what you could not have prevented
In most cases, the cause is unknown. That sentence does a lot of quiet work for owners who spend the weeks after a diagnosis auditing everything they ever fed their dog. The white paper reports that studies have found links to herbicide exposure, genetic predisposition, second-hand smoke exposure and viral infections, but it stops well short of naming a cause for any individual case. In cats the picture is clearer for one specific route: cats infected with feline leukaemia virus or feline immunodeficiency virus have a significantly higher rate of lymphoma than the general cat population, and vaccination against feline leukaemia virus has meaningfully reduced that risk.
For dogs, no equivalent single lever exists. The most useful thing you can do is notice changes early and act on them.
Where the research is heading
Morris Animal Foundation reports investing almost $4.5 million in more than 50 lymphoma studies over the past 30 years, plus more than $32 million in its Golden Retriever Lifetime Study, which follows over 3,000 dogs across their lives and treats lymphoma as one of its primary endpoints. Three strands of that work are worth knowing about.
- Immunotherapy. In one recent study, researchers extracted T cells from dogs with lymphoma, genetically modified them to improve their cancer-killing ability, grew them in the laboratory and returned them to the dog. Early results are described as promising, with more work needed before anything reaches the clinic.
- Earlier detection. A study is searching for early signatures of lymphoma in the blood of Golden Retriever Study dogs. If it works, the paper suggests it could become part of a routine bloodwork panel — turning lymphoma into something found before it is felt.
- New drug targets. Foundation-funded researchers have identified promising targets in B-cell lymphoma and are testing drugs against them in the laboratory, with similar work under way for other canine lymphoma types.
None of that helps a dog diagnosed this month, which is worth being honest about. What helps this month is a prompt appointment, a fine needle aspirate, and a frank conversation about subtype and options.
